110-301-0106(3)(a-b) staff member on-site did not have her school age basics training completed. 110-301-0106(4) both staff members on site did not have training completed at the time of the visit. 110-301-0106(5) both staff members on site did not have training completed at the time of the visit. 110-301-0106(6) both staff members on site did not have training completed at the time of the visit. 110-301-0106(8) staff memeber did not have blood born pathogens training completed at the time of the visit and certificate states 3/28/2024. 110-301-0106(10) staff member food handlers card was expired at the time of the visit.
