0165 (4f) Licensor observed the stairway leading to the unlicensed space (2nd floor) was not secured with a safety gate making the unlicensed space accessible to children in care. (immediate corrections: safety gate was placed at the bottom of the stairway). 0165 (5a) Licensor observed 2 outlets in the child care space was not tamper-resistant or covered with a blank plate. 0165(5b) Licensor obsered the outlet near the kitchen sink was not GFCI outlet type.
