Observation

An epi pen was observed with no prescription box to indicate who the prescription was for. No medication authorization form was accompanied with the medication or information that would indicate possible side effects of the medication or how to give the medication. The child's first and last name was written on a zippy bag only. No date observed when the prescription was filled or contact info of the prescribing health care professional. No written consent signed consent observed with an epi pen medication observed in the classroom. No medication label as the medication box was missing.
