110-300-0165(2)a) Licensee had tacks that were accessible in the school age room that pose a risk of chocking or ingestion. 110-300-0165(2)(c) Licensee has cords accessible in the Annex room to a tv, vcr, cords to a computer in the storage area in the back of the room, a cord was accessible to the a small refrigerator on the back deck area, cords in the toddler room under the sink area, and cords under the changing table to Christmas lights all cords were cable of forming a loop around a check neck. 110-300-0165(2)(d) licensee had plastic bags in the Annex room, under the sink area. 110-300-0165(3)(a) Licensee had parchment paper box with a razor was accessible in the Annex room drawer. 110-300-0165(3)(g) Licensee had chipping paint in the Annex room in the bathroom area green shelf, licensee has chipping paint on the deck by the Annex room, licensee has a couch and chair with rips in the Annex room and broken wrippted box.
