Observation

1)a) One child's individual care plan had a diagnosed medical condition and two other diagnoses (not medical issues) on the plan. There was information in the plan regarding one of the diagnoses, but there were no instructions, plan, or information about the child's medical condition on the individual care plan. The three diagnoses/issues on the plan were different types of issues that would require different plans. 1)a) Another child's individual care plan has information in the section regarding medication filled out with something that is not needed for the child's diagnosis during program. 1)a) Another child with an emergency medication for a diagnosed health condition did not have an individual care plan on file. 1)a) An individual care plan for a child's serious health condition did not have clear instructions for staff regarding how they need to monitor the child's condition. The plan included instructions for medication to give if the child's condition worsened, but the information regarding how to test when the child's condition is worsening is missing. The child usually carries a cell phone or device to show the numbers to indicate if medication is needed, but that cell phone or device is currently broken and staff do not have a plan for this situation.
