3)a)i)A)B)C)D) A child's prescription medication was in a plastic bag with a sticker on it that the child's name had been written on. It did not have the prescription label with the child's name, date prescription was filled, name and contact information of the prescribing health professional, expiration date, dosage amount, length of time to give the medication, instructions for administration and storage, or information regarding possible side effects. 3)a)i)D) The medication authorization for a child's prescription medication had an end date of 8/22/2025. A new medication authorization has not been completed and the medication has been dispensed by staff since the end date of the last authorization. 3)a)i)A)B)C)D) A prescription emergency medication did not have a medication authorization completed by the parent and was not labeled with a prescription label with child name, health care provider name and contact info, information regarding side effects, expiration date, dosage amount, length of time to give the medication, or instructions for administration and storage.
