Right At School at Harvard Elementary

Observation

During the onsite monitor visit this licenor observed a medication of a epi-pen that did not have a prescription label on it with child's name, date prescription was filled, contact information of the health professional,expiration date, and instuctions for storage. This licensor also did not observe a medication authorization form with the medication that states side effects and medical need.
