Observation

0260(1)(a): After interviewing staff and the referent, more likely than not the changing table had wet disinfecting bleach on it when the child was laid down for a diaper change. The staff member reported smelling bleach but noticing the changing table wet. The referent provided a picture of the child's shirt with bleach staining on the back of the shirt. Licensor was able to verify via video footage that the picture of the shirt matched the shirt the child was wearing at pickup on the day of the allegation.
