0215 (2) Licensor observed staff were not trained by the child's parent for special medical procedures that are part of a child's individual care plan. 0215 (3) Licensor did not observe a medication consent form for a child in need of a life saving medication (EPI PEN). Licensor observed diaper cream ointment was provided on multiple children without the parent consent form signed or completed.
