Observation

No individual care plan from a child's health provider was provided to the program for children with known allergies. Additionally, no form identifying substitute foods for allergy-related dietary restrictions was obtained from the parents. The files lacked directions and dosage information for medication prescribed to an 18-month-old child, despite a note on the medication box stating "not to be administered to children younger than 2 years of age."
