0186 (1) Licensor observed a child's file with a known food allergy indicated on the enrollment form did not have the supporting documents (Individual Care Plan) completed or signed by the parent or the child's health care provider. 0186 (2) Licensor observed there was no supporting documents (individual care plan) if the child with a known food allergy was prescribed the necessary medication.
