110-300-0210(2)(a)(c)-Licensor observed 5 child files, 2 of the files had immunization forms from out of state (both Califonia) and 3 immunization forms were outdated (1 was an outated form missing parent signature and 2 forms had small print outs of immunizations that children had received).
