0165(2)(d) Diaper changing table in the restroom was wrapped in a large plastic bag at reach and accessible to children. Plastic bag was removed at the time of visit. El plastico se retiro al tiempo de la visita. 0165(3)(c) Activity table in the outdoor licensed área had cracked plastic cover. Table was removed at the time of visit.
