Observation

A child has a list of food allergies but did not have paperwork for all the allergies. There was only a Individual Health Care Plan for Peanuts. The Individual Plan was incomplete and lacked the signature of the doctor on the plan for the Epipen. The licensor also did not observe a medication authorization form for the Epipen. It was not clear if all the allergies were as severe as Peanuts but there is a list of food allergies and Peanuts was the only one listed on the incomplete plan.
