0186 (1) Licensor observed a child with a food allergy was not completed or signed by the child's health care provider or parent with a known food allergy. 0186 (2) Licensor observed there was no training with staff to properly manage a child's food allergies. 0186 (4) Licensor observed the Individual care plan information was not reviewed due to no parental/physicians signatures on the Individual care plan.
