110-300-0165(2)(b) Licensee has a mini blind with cords in Chapel room A2, and one mini blind with a cord accessible in the bathroom in D-6. 110-300-0165(2)(c) Licensee had cords accessible in the chapel room to a TV cord that could form a loop around a child’s neck, in room D-6 five-year-old classroom there was cord to the refrigerator accessible, in C-5 a cord to the heating element that is accessible that could also form a loop around a child neck. 110-300-0165(2)(d) Licensee has plastic bags accessible in the Preschool room D-6 in the bathroom area and hanging in the bathroom on the wall in room B-4. 110-300-0165(f) Licensee had a humidifier and a mini refrigerator in room D-6 that poses a risk of tipping, in B4 a red shelf, and a bookshelf posing a risk of tipping, a large TV in in room C-5 that poses a risk of tipping and in room B-3 there was a large seven shelf that poses a risk of tipping, a kitchen set, a white shelf and two wood shelves that all pose a risk of tipping that need to be secured to the wall. 110-300-0165(3)(g) Licensee has chipping paint on the water table, chipping pain on the gate and on the portable exterior wall in the playground for 4s and 5 years.
