0166(3)(a) The exit door in middle room was locked. It was then unlocked during the visit. 0166(3)(c) Exit door in middle room had high chair in front of it. Provider moved it out of the way during the visit. 0166(4)(b) The back room does not have an approved second exit because the window is too high off the ground and too small.
