Observation

Some windows observed to have openings more than 3.5 inches. Window fans observed in 2nd story windows. Fans are not secure and could fall if pushed. Windows were closed and locked, fans were removed, and windows closed and locked. One mirror without tempered glasses observed accessible to children, mirror used for children to observe themself. Provider removed mirror, she reports she will replace it with mirror with safety glass.
