Observation

Some diaper ointment consents are missing information from the medication authorization form (amount, when to apply expiration date, child's DOB, end date). A child had a Tylenol medication authorization form that was missing information which includes why the Tylenol is being give to the child. Medication/Diaper Ointments were missing last name. In the Preschool Room there was sunscreen under the bathroom sink and the cabinet was not latched which allows children access.
