On 10/27/23 at the on-site inspection the following was noted: potty seat used on the toilet on bathroom floor has visible dirt on it. Flooring it toileting area has water damage and separating. Flooring surface cannot be properly cleaned due to water damage. On 10/27/23 there was not soapy water available to clean the diaper changing pad
