During onsite inspection, Licensor reviewed five child files and requested to review Provider RE’s biological child’s record and observed that one child’s immunization was past due for MMR (10/24/21), DTaP (03/08/23), and Varicella (10/24/21), and no Department approved COE was in child files, and no immunization record onsite for biological child.
