It was observed that the electronic workforce registry (MERIT) was not regularly maintained and entered at the time of inspection. An assistant's (OG) record/file was not established at the time of inspection. An assistant's (ME) record/file was observed to be missing the following items at the time of inspection: item (a) - item (e). / The provider and two assistants' (RG & JL) record files were observed to be missing the following item at the time of inspection: item (b).
