0165(2,a): licensor observed in the changing room for the toddlers, Styrofoam in a box on the floor and accessible to children. 0165(2,d): licensor observed plastic bags in children's individual storage in the preschool room and the outdoor space. 0165(3,a): licensor observed the door to the kitchen opened and adjacent to the preschool classroom in which a sharp object was on the counter and accessible to children. 0165(5,a): licensor observed an electrical outlet in the toddler room not have automatic shutters, or covered with a blank plate, or tamper resistant.
