110-300-0165(2)(d)- In the Navigator classroom, there were plastic bags in the bathroom that were accessible to children. 110-300-0165(3)(a)- The playhouse on the playground had a broken piece that resulted in a sharp edge. The chain link fence has pulled up from the ground leaving the fence wires exposed. Some of the wires were sharp. 110-300-0165(5)(e)- In the Voyager classroom, there was a power strip being used that did not have a surge protector.
