3)a)i) There were prescription medications for children that did not have medication authorizations for each medication. They were both listed on the same authorization and didn't have specific dosage information for each. 3)a)ii)A) Non-prescription medication was not labeled with child's first and last name. The medication authorization for the medication did not have the expiration date, medical need, dosage amount, and length of time to give the medication. The authorization also had two prescription medications listed.
