3)a)i) One child has a prescription medication that does not have a medication authorization completed for staff to dispense the medication. There are instructions from the health care provider, but no authorization completed by the parent. 3)a)i) There is another child with a medication authorization for a prescription medication that is not filled out correctly as there is no dosage information and there are two medications listed on the authorization for two separate medical issues. One of those medications is not on site. 3)a)ii)A) One child has a non-prescription medication. The medication authorization is not filled out correctly and it is not clear when or why medication is needed.
