0145 (11) At the time of the visit, the licensor observed that gate does have a self-closing and self-latching mechanism. 0145 (6) At the time of the visit, the licensor observed that outdoor play area is not fully enclosed with fence, the side yard is open. 0145 (8) At the time of the visit, the licensor observed gaps of more than 3.5 inches on some areas of the fence.
