Observation

An epi pen for a child with an identified food allergy, was missing the prescription box identifying who prescribed the medication and missing contact info of the prescribing health care professional. The prescription medication was not labeled with the first or last name of the child or the date the prescription was filled. No dosage amount or the length of time to give the medication or instructions for medication storage observed. Only one epi pen present with no indication of who it was prescribed to.
