0165(2)(d) There was some plastic bags in the cubbies and in the kitchen during monitor visit. 0165(2)(g) Playpen was placed under window during the monitor visit. 0165(3)(a) There was some sharp knives accessible in kitchen closed drawers and some razor in the shower during the monitor visit. 0165(3)(c) There was a bike missing plastic handlebars in the outside license area. 0165(3)(f) Provider had shelves that need anchored to avoid any tipping hazard 0165(5)(d) There was an extension cord being used in the living room that was not surge protector. 0165(2)(c) There were some cords next kitchen counter area not secured leaving cords accessible during monitor visit.
