0165(2)(a) There was plastic covering a mattress in one of the cribs. 0165(3)(a) There were three nails exposed on the upper part of the outdoor play area. Pricker bushes were sticking out through fence. The mirrored light switch cover in the bathroom was partly broken and sharp. 0165(4)(f) The outdoor railings had gaps larger than 3 1/2 inches. 0165(4)(g) There is a raised platform grassy area of the yard that is accessed via stairs and drop zone has no railing.
