During onsite inspection, Licensor requested to review facility site records, and Provider was unable to produce records of the State Fire Marshal form for current year or previous licensed year. (1) During onsite inspection, Licensor requested to requested to review facility site records, and Provider was unable to produce State Fire Marshal form, monthly testing of smoke detectors and carbon monoxide detector, and monthly fire extinguisher and fire hazard inspection.
