0210(1)(2) Licensor observed child 1 immunizations is conditional, child 2 immunization is not complete, child 3 immunizations is not complete and child 4 is missing immunizations. 0210(3) Licensor observed parents or guardian has not provided signed and dated statement detailing when the child's immunzations will be up to date and stating they understand their child will be excluded from care if the immunizations are not completed within 30 days of the specified date.
