0165(2)(c) in the hallway, an iPad charger cable is accessible that could form a loop around a child's neck. 0165(3)(d) the washer and dryer are accessible that are entrapment hazards. 0165(3)(g) the following items were in poor condition: some chipping paint around the facility walls (toddler and preschool rooms), children's sofa and green cushions - library section is peeling and torn, carpet trim has duct tape (preschool room), and foam is deteriorating/plastic trim peeling (toddler room).
