110-300-0165(2)(d)- There were plastic bags accessible in the toddler and pre-toddler classrooms. The provider moved the bags out of the reach of children at the time of the visit. 110-300-0165(3)(a)- In the preschool bathroom, there were three toilets. At the base of all three, there were screws extending up from the base that were not covered or cut down to a short length. 110-300-0165(3)(g)- A section of base board in the transitional preschool room was removed. The unfinished wall and a small hole with jagged edges were exposed.
