During onsite inspection, Licensor requested to review Washington State Fire Marshal form, and Provider IE was unable to produce (stating locked in bedroom and didn’t have key). (2) During onsite inspection, Licensor did not observe a minimum of a 2A:10 BC fire extinguisher mounted and installed to meet the State Building Code, and later observed a person arrive and drop off a fire extinguisher to be installed.
