Observation

During onsite inspection, Licensor observed two water bottle caps on the floor in the licensed play areas, and used a choking device and measure them less then one and three-quarters in diameter and posing a risk of choking with two toddlers present in care. (2)(a) During onsite inspection, Licensor observed cords to a play guitar, iPhone/iPad charge, space heater, TV, extension cord, and cable cords capable of forming a loop and accessible to children in care. (2)(c) During onsite inspection, Licensor observed a black plastic trash bag on the floor in the dining area with laundry and later on the child sized table accessible to the two children present. (2)(d) During onsite inspection, Licensor observed an extension cord being used as a main outlet for iPhone/iPad charger, television, and cable outlet in the licensed area. (5)(f)
