3) One medication authorization did not have the parent signature on the authorization form. 3)a)ii) There are medication authorizations for non-prescription medication in children's files. The authorizations are each missing one or more pieces of the information required by this rule, including expiration date, medical need, clear dosage information, and/or length of time to give the medication. The medication on site identified by the provider as belonging to a child in care was not labeled with the child's first and last name. There were some medication authorizations on file for medications that were not available on site. 3)c)i) There were several diaper rash ointments in the restroom that were accessible to children in the open bins on the diapering station. The provider removed them during the inspection.
