110-300-0210(1)(2) - Upon review of children files, licensor observed that 11 children do not have a current, complete, and medically verified certificate of immunization status (CIS) form, a department approved certificate of exemption (COE) form, or a current immunization record form the Washington state immunization information system (WA IIS). These immunizations are "Not Complete" or "Conditional" and do not have parent signatures with date.
