Provider's facility site on MERIT was not kept up-to-date at the time of inspection (Provider's completed Bloodborne pathogen training was not entered into the system). Staff files were observed to be missing the following items at the time of inspection: A resume and an emergency contact information (HD), a resume and a government issued photo ID (SS).
