0165(2)(c) There were some cords not secured to the wall during monitor visit. 0165(2)(d) There were some plastic bags in the kitchen and in the art supplies. 0165(3)(c) There was a knife in the top-drawer kitchen that was unlocked. There was a hole in the free playroom with sharp edges. 0165(3)(c) There were some cracked outlet covers in the center. 0165(3)(g) There was a mirror with flaking paint in the restroom upstairs. 0165(5)(d) There was an extension cord being used in the center room that was not surge protector.
