Observation

At the time of the Monitor Visit, there were plastic bags accessible to children in the storage room. At the time of the Monitor Visit, the wall paint above the diaper changing station, near the outlet, was flaking. At the time of the Monitor Visit, the bathroom outlet was not GFCI, and the outlet cover was not secure to the wall.
