(2)(c) in the blue room there was a radio cord forming a loop in the quiet area. (2)(d) In the red/ toddler room there were plastic bags in the bathroom low enough for accessibility. (3)(g) In the red room there was flaking paint and equipment in poor (puzzles/ toys, file cabinet, cubbies) condition with loose paint.
