During onsite inspection, Licensor did not observe placement markers at children’s eye level on the glass sliding glass door. (2)(f) During onsite inspection, Licensor observed blackberry bushes posing a risk of cutting/puncturing and a stack of wooden materials accessible in the outdoor play area. (3)(a) During onsite inspection, Licensor observed a set of cubbies not secured or anchored to keep from tipping over. (3)(f) During onsite inspection, Licensor observed the bulb in the bathroom was glass and not made of shatter resistant material. (4)(c) During onsite inspection, Licensor observed an outlet 11 inches from the washer water hose hook up, and an outlet 18 inches from the bathroom handwashing sink not equipped with a GFCI outlet type or screw in blank plated.
