0165(2)(d) Licensor observed plastic bags, gloves and wipes were accessible to children in the bathroom. 0165(3)(c) Licensor observed a broken plastic lid of a plastic tote in the outdoor play area that posies a hazard of pinching and accessible to children. 0165(3)(g) Licensor observed the climber in the outdoor play area that had rusty bolts, a rusted rack in the bathroom and a rusted can and water can, in the back outside play area accessible to children. 0165(4)(e) Licensor observed water temperature in the bathroom is 127.3. I have put away the plastic bags, gloves and wipes in the bathroom, removed the rusty rack in the bathroom. I threw away the broken plastic lid and the rusty items outside the playground. I adjusted the water temperature under 120 degree in the bathroom.
