0165(2)(c) There was a phone cord capable of forming a loop not secured to the wall near the top shelf during the monitor visit in the preschool and school age classrooms. 0165(2)(d) There were some plastic bags hanging on the door hook in the restroom and in closed cabinet with art supplies during the monitor visit.
