2)c) There was an extension cord inside the door of the barbecue grill in the outdoor play area. 2)d) There was a plastic bag in an unsecured drawer in the bathroom. The bag had cotton balls in it, but was large enough to pose a suffocation hazard to a child. 3)a) There was a Fisher Price riding toy in the outdoor play area that had a screw sticking out from the front that had duct tape loosely around it. The riding toy also had a protruding plastic key that was the subject of a recall of this riding toy several years ago. 5)a) There was an electrical outlet on the back of the house that is accessible to children and is not tamper resistant or covered.
