Observation

One child did not have an enrollment record at all. One child's file was missing date of last physical exam. Two children's file did not contain dental provider information. One child's file did not list any emergency contacts or pick up persons. Three children's file were missing date of last dental exam. One child's file did not include authorization for provider to seek first aid/emergency treatment for child care licensee or qualified staff. Children's health record and the information in subsection (2) (a) through (e) was not available to staff for medical adminstration or emergencies.
