Observation

(2)(a) Small people blocks in the young toddler room were less than 1 3/4 inch in diameter. These were removed during the site visit. (2)(d) Plastic bags in an unlocked/unlatched closet were accessible to children in the operating preschool room. These were removed during the site visit. (3)(a) A large mirror was propped up against cots in the school age room and posed a falling or cutting hazard. A metal bracket by the gate to the bathroom in the toddler room had sharp edges. Some bolts on the playground fence were protruding (more than two threads). Metal brackets/clips on the fence were sharp. (3)(c) A fan was accessible to children in the operating preschool room. The fan was removed during the site visit. (3)(e) A piece of material on the ground in the school age playground was pulled up and posed a tripping hazard. (3)(g) Drinking fountain in the Pre-K room was missing a part on the side exposing the inside of the fountain. (4)(3) Water temperature at the hand washing sink in the Pre-K room was 122 degrees f.
