0165(2)(d): At the time of inspection, it was observed that there is a plastic bag hanging on kitchen cabinet which was accessible to children. 0165(3)(a): At the time of inspection, it was observed that there are mutiples places on playground has sharp end of the nails exposed to children. 0165(5)(a): At the time of inspection, it was observed that the electrical outlets do not have automatic shuters and temper resistant. 0165(5)(b): At the time of inspection, it was observed that electrical outlet in bathroom is not tamper resistant and GFCI outlet type.
