3)a)i) A child's prescription medication and a non-prescription oral medication, for use in emergency, do not have medication authorization forms completed. There is a document from the school with instructions from the doctor, but no medication authorization from the parent giving staff permission to dispense the medication. 3)a)i) A child's prescription medication has a different dosage on the label than what is showing on the medication authorization and individual care plan.
